Why it matters

Cash flows from operations were $5.1 billion, or 1.2x net income. The debt-to-capital ratio was 41.7% as of September 30, 2026. Operating earnings rose $2.3 billion from $4.3 billion a year earlier. The operating cost ratio was 12.8%, compared with 13.5% in the third quarter 2025. MCR was 88.4% and included $610 million of net favorable prior period reserve development related to 2026 dates of service.

OptimistFi's case is that UnitedHealth's scale still turns revenue into cash and shareholder returns, and the raised outlook supports that view. The caveat is that Optum Health revenues of $22.9 billion decreased 10% year-over-year. That keeps the normalization question open for the services business. UnitedHealthcare earnings from operations were $2.8 billion, and Optum earnings from operations were $3.7 billion.

UnitedHealthcare revenues were $86.3 billion, while Optum revenues were $66.9 billion, keeping both businesses central to the quarter. The company repurchased $5.0 billion of common stock and paid $6.2 billion in dividends year-to-date.
